Documents and forms that you will find useful in the college process. The resume, self-assessment form, and autobiography need to be turned in to your CAC counselor by 3/1 before you can schedule your first appointment.
Junior Forms:
- The Self Assessment Form (DOC, 52k):
- This vital document gives us a snapshot of your overall experiences both at your home school and at IMSA. It is one of the pieces of information we use when writing your letter of recommendation.
- Autobiography: This is a confidential (usually 4-6 page) document for your CAC’s eyes only.
- Be sure to include:
- The circumstances of your birth,
- Your place in your family,
- A bit about your childhood and early schooling,
- Much more about your high school years, and
- Your aspiration.
- Be sure to weave with the facts:
- Your thoughts,
- Your beliefs, and
- Your feelings/emotions.
- Put YOURSELF on paper
- Be sure to include:
- Resume: (sample resume (DOC, 32k))
- One page (back to back) document highlighting your activities, awards/honors, etc.
- Activities listed should be from your high school years only UNLESS it is an activity you started prior to high school but are still doing now (e.g. piano etc.).
